Populate all required fields.  Then save the form in Excel or .cvs format and email it as an attachment to finance@governmentresource.com.

An invoice will be emailed to you.  Please note that a $20 manual billing fee will be applied to all "Bill Me" orders.

Upon receipt of payment, participants will be uploaded into the Pay-Per-View system.  Each participant will be emailed the access information for the course(s) for which they have been registered.
